1) Consent to a CORI (Criminal Offender Record Information) check

2) Sign a Recreational and Volunteers Activities Release Form

3) Provide a copy of their Driver’s License (see page 2 of the CORI for acceptable forms of ID)

4) Provide a copy of their COVID-19 Vaccination Record Card (optional)
